چکیده

Nineteen mtrahepatic islet allografts were performed in 18 patients. In group I. nine patients aged 8 to 58 years underwent (ombined liver-Islet allotransplantation following upper-abdominal exenteration for tumors too extensive to be removed with less drastic procedures.b•7 Preliminary results on these patients have been reponed previously.~ In group 2. three type I diabetic patients aged 22 to 56 years received a combined liver-islet allograft. The indications for liver transplantation were cirrhosis secondary to hepatitis C. alcoholic cirrhosis. and cryptogenic cirrhosis. All patients had an absent C-peptide response to glucagon or Sustacal challenge test. In group 3. six patients aged 28 to 42 years received seven combined cadaveric kidney-islet grafts (one retransplant) for end-stage renal disease secondary to type I diabetes mellitus. The human islets were obtained by a modificationM of the automated method for human islet isolation. ~ The preparation was Infused into the ponal vein catheter over 20 to 30 minutes. Immunosuppression was with FK-506 (group Il and FK-506 plus steroids (groups 2 and 3), Supplementary sterOIds or OKT3 was given if rejection was suspected clinically or diagnosed by biopsy.

Outcome of human islet isolation and allotransplantation in 20 consecutive cases.

This report provides our initial experience on islet isolation and intrahepatic allotransplantation in 20 patients. In Group 1, 10 patients underwent combined liver-islet allotransplantation following upper-abdominal exenteration for cancer. One patient underwent pancreatic islet allograft after near total pancreatectomy for chronic pancreatitis. In Group 2, 3 Type I diabetic patients received ...

Histologic finding of pancreatic islet tissue following intraportal human islet allotransplantation.

THE ultimate test of viability of separated human islets is functional activity after transplantation into a diabetic recipient. Morphologic identification of insulin-containing transplanted islets could provide important correlative data and eventually confirm the functional results. The present report describes identification of intrahepatic islet tissue following human islet allotransplantat...

Successful treatment of brittle diabetes following total pancreatectomy by islet allotransplantation: a case report.

CONTEXT Allotransplantation of islets can successfully treat subjects with type 1 diabetes complicated by severe hypoglycemia and erratic glycemic control. Insulin independence is often lost over time due to several factors, including recurrent autoimmunity. Brittle diabetes (frequent hypoglycemia and labile glycemic control) is common after pancreatectomy. This is ameliorated by auto-islet tra...

Islet cell allotransplantation in diabetic patients. Histologic findings in four adults simultaneously receiving kidney or liver transplants.

Refined methods of islet cell purification have led to unprecedented success of islet cell allotransplantation via portal vein infusion in diabetic patients, resulting in marked reduction of exogenous insulin requirements and recently even insulin independence. The authors report the histologic findings of islet cell allografts in the liver of four patients who had undergone combined kidney-isl...

Prospective and challenges of islet transplantation for the therapy of autoimmune diabetes.

Pancreatic islet cell transplantation is an attractive treatment of type 1 diabetes (T1D). The success enhanced by the Edmonton protocol has fostered phenomenal progress in the field of clinical islet transplantation in the past 5 years, with 1-year rates of insulin independence after transplantation near 80%. Long-term function of the transplanted islets, however, even under the Edmonton proto...
